Intake Process

There will be some screening questions that will be asked prior to your first appointment. This will help us determine if we can safely and fully address your concerns. These questions will include a list of your current medications. It is also helpful to ask your referring provider to fax us a formal referral. Please work to obtain any prior pertinent medical or psychiatric records prior to your intake appointment. In some instances, we may require those records prior to scheduling an appointment.

Intake Forms

There will be a set of intake forms that will need to be completed prior to your intake appointment. These forms can be completed via confidential secure online forms, via confidential email or by paper packet and will need to be completed prior to scheduling your intake appointment. We will also need a copy of your valid ID and insurance card.

Insurance

We accept and are preferred providers with several medical insurance plans:

Premera, Blue Cross/Blue Shield, Federal Blue Cross/Blue Shield, Aetna, Meritain, Moda, Tricare, Triwest/VA Community Care Network, United Healthcare Commercial, and Providence Behavioral Health.

Several local employers/health trusts partner with the Aetna network for preferred providers such as EBMS, ASEA, Alaska Electrical, and Alaska Pipe Trades. Our office is preferred for these insurance plans since we are Aetna preferred providers.

Our office will bill, on your behalf, most private insurances and most out of network insurances.

We also accept self-pay patients. All self-pay clients are required to pay at the time service is rendered and may receive a same-day payment discount.

Some insurances have a “carve out” for mental health care. This means their mental health services are contracted through a different health insurance company. If we are not currently in-network with that “carve out,” we are considered out of network for those patients and the insurance company may determine the patient is responsible for the full fee for services.

We are not currently accepting Medicaid/Denali Kid Care.

Please inquire with your insurance carrier directly regarding any prior authorizations, deductibles, co-pays, or coinsurance prior to scheduling an appointment.

Keeping Appointments

Please make every effort to attend your appointments.  We require one full business day (24 hours) notice for cancellations.  Monday appointments need to be cancelled by 5 pm the preceding Friday. Please note that insurance will not reimburse missed or cancelled appointments and no show fees may apply. Please note that missed intake appointments will not be rescheduled.
